Patients are seen by appointment – on the same day the appointment is made if possible. To schedule an appointment, please call (603) 434-7444 during regular business hours.

Please call in advance for non-urgent appointments so that we may schedule a date and time to fit your convenience. We recommend that you schedule your next follow-up appointment at the conclusion of your office visit.
Please bring your health insurance card to your initial visit, as well as subsequent visits. If there is another responsible payer involved, such as automobile insurance or workers’ compensation insurance, you will be asked to provide the name and claims address of the party responsible for payment. In addition, please bring:
- a referral from your primary care physician if necessary
- a photo ID, such as your driver’s license
- a list of the medications you are taking
- a list of known drug allergies
- relevant medical records and reports
- recent imaging tests, such as x-rays and MRIs
- any forms you were asked to bring for your visit
New patients are asked to arrive 15 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of x-rays, lab results and your insurance card to your appointment.
We request and appreciate payment at the time of service for the portion of the fee for which you are responsible (e.g. co-payments, deductibles, and past-due account balances.) Statements are mailed monthly. We accept most major credit cards for your convenience.
Our staff will make every effort to simplify all billing and insurance claim procedures. We participate with many insurance plans; however, we recommend you call your insurance company prior to your visit to verify coverage and to determine if a referral from your primary care physician is necessary. Please bring your insurance card with you to every visit so that we may ensure that our records are accurate. If you do not have insurance coverage, full payment is required at the time of service.

Please note that “Medicare replacement” plans, also called “Medicare Advantage” or “Part C”,plans, are provided by private insurance. If you enroll in a Medicare replacement plan, the plan,replaces your original Medicare and may not include your doctor. Please check with your,insurance to determine if our physician is included in your plan.
If you do not see your insurance listed above, please call the number on your card to obtain benefit and coverage information.
Many insurance companies require you to obtain a referral in order to see a specialist. Please make sure you check with your insurance company and/or primary physician before your scheduled appointment to see if a referral is needed. If a referral is needed it is the patient's responsibility to bring the referral to your scheduled appointment. Your appointment will be rescheduled if your referral is not obtained.
If you have had any testing (MRI, X-Rays, CT scan) done, please bring a copy of the actual study and report. You may pick up these films or CD's at the imaging center where the test was performed. Please call the imaging center 24 hours in advance to have your films or compact discs ready.
Cancellations or Rescheduling
We call to confirm your appointment one business day prior to your planned visit. In the event that you must reschedule an appointment, please notify our office at least 24 hours in advance.

After-Hours & Emergencies
If you are experiencing a medical emergency, dial 911. However, if you need after-hours advice for a medical concern, please call (603) 434-7444 and our answering service will assist you.
